A case of extended cutaneous basal-cell cancer in the periauricular region that affects the temporal bone and produces metastases in cervical lymphatic nodes is presented. The patient has been ill during 7 years. The initial treatment of the tumour included multiple cryodestruction with short periods of remission. The patient did not give consent to surgical intervention. The progress of the tumour necessitated the referral of the patient to the Research Oncological Institute where he was given radiotherapy with a good clinical outcome. However, the recurrence of tumour growth after 3 years resulted in its spread over the parotid gland, paresis of the inferior branches of the facial nerve, and the destruction of the external auditory canal. Radical surgery included parotidectomy, resection of the external auditory canal, and the affected temporal bone within healthy tissues combined with cervical lymphodissection. The radiotherapeutic treatment was repeated in the postoperative period that was otherwise uneventful. The healing occurred by first distention. To date, remission has lasted for 19 months.
