Abstract:

OBJECTIVE

To study the incidence of myocardial reperfusion syndrome after surgical revascularization.

MATERIAL AND METHODS

The study included 556 patients with coronary artery disease and occlusive coronary lesions who underwent myocardial revascularization. All patients were divided into three groups depending on postoperative left ventricular systolic function. Analysis involved the second and third clinical groups, since there was no systolic dysfunction in the first group. In each group, ECG was analyzed (one-time registration and daily monitoring). The «classic» ECG criteria for myocardial infarction were considered. Results were statistically analyzed in the R Studio environment.

RESULTS

ST segment elevation was found in all patients of the third group and most patients of the second group. Reciprocal ST segment depression in any two adjacent leads was observed in 17.4% of patients of the second group and all patients of the third group. Q wave at the end of the first week occurred in all patients of the third group and was absent in all patients of the second group. Deviation of electrical axis was noted in 27 (23.5%) patients of the second group and 12 (80%) patients of the third group.

CONCLUSION

Patients with persistent postoperative systolic myocardial dysfunction had all ECG criteria of myocardial infarction. In patients with transient LVEF decrease, there were «mosaic» ECG criteria for myocardial infarction. This did not allow us to confirm myocardial infarction in this group. Statistical analysis confirmed heterogeneity of these groups in ECG criteria.
