Today, the Bentall procedure is the most common procedure for aortic root pathology, but it is not without drawbacks. Modern aortic root surgery pursues the following purposes: radicality, continuation of life and quality of life improvement. Various valve-sparing procedures, as well as valve-replacing surgeries have own advantages and disadvantages. The aim of this review was to determine competitive advantages of each procedure on the aortic root and develop surgical algorithm. We analyzed aortic root surgeries devoid of prosthesis-induced drawbacks. Ross procedure and «Russian conduit» were also considered in case of severe valve dysnfucntion or structural lesion.
