Abstract:

We present a patient with atrial fibrillation and thromboembolic complications manifested by thrombosis of the left atrial appendage while taking oral anticoagulation (rivaroxaban 20 mg), as well as recurrent ischemic cerebrovascular accidents (dabigatran 150 mg 2 times per day). The left atrial appendage was successfully occluded in this patient, and low molecular weight heparin was administered (enoxaparin 80 mg per day) for 45 days. Considering additional risk factors of thromboembolic complications (residual blood flow 5 mm, reduced left ventricular ejection fraction and increased D-dimer), we continued anticoagulation that prevented thromboembolic complications in long-term follow-up period.
