The study included 622 patients after coronary artery bypass surgery including 39 cases in combination with carotid endarterectomy. All patients were divided into 2 groups. The first group included 169 patients with low risk (Euroscore index 1-2 points). The second group included 453 patients with high risk (Euroscore index more than 3 points). 257 patients were operated by using of cardiopulmonary bypass and pharmaco-cold cardioplegia. Coronary artery bypass surgery with parallel cardiopulmonary bypass was done in 101 patients. Coronary artery bypass surgery without cardiopulmonary bypass was done in 264 patients. It was observed that none of the methods of CABG has no advantages in patients with low operational risk. Coronary artery bypass surgery with parallel cardiopulmonary bypass lead to decreasing of perioperative acute heart failure frequency in patients with reduced pumping function of left ventricle. The refusal of cardiopulmonary bypass leads to decreasing of acute respiratory failure frequency after surgery and the need for prolonged mechanical ventilation in patients with chronic obstructive pulmonary disease. This tactics reduces the residence time in the coronary care. Coronary artery bypass surgery without cardiopulmonary bypass is accompanied by a decreasing of intraoperative blood loss volume and significant reduction of cerebral complications frequency in case of hemodynamically significant lesion of the brachiocephalic arteries. The refusal of cardiopulmonary bypass in patients with chronic renal failure leads to decreasing of intraoperative blood loss volume. Also it preserves an impaired renal function in higher degree in comparison with surgery under CPB.
