Abstract:

Results of treatment were analyzed in 280 patients with carotid atherosclerosis and prior ischemic stroke (IS). Among them 195 (69,7%) patients (group I) underwent surgical cerebal revascularization, 85 (30,3%) patients (group II) were not operated despite indications for operation. Patients from the group I were additionally divided into subgroups depending on the terms of operation after IS episode. It is shown that carotid endarterectomy is effective in improvement of cerebral functions and decrease of risk for recurrent IS in this group of patients. The most optimal term for surgical brain revascularization for achievement of the best clinical effect and improvement of life quality is the period from 1 to 6 month after IS. Operations carried out later (more than 1 year after IS) have mostly preventive that curative significance.
