OBJECTIVE

To determine clinical and hemodynamic efficacy of radiofrequency catheter ablation (RFA) for ventricular tachycardia (VT) and effect of interventional treatment of VT on left ventricular (LV) systolic function in patients with chronic heart failure (CHF) with reduced and moderately reduced LV ejection fraction (EF) within 12 months after intervention.

MATERIAL AND METHODS

RFA of ventricular tachycardia was performed in 47 patients (41 men (87.2%), 6 women (12.8%), median age 65 [57; 71] years) with ischemic CHF NYHA class II—III, LV EF <50% and persistent paroxysms of monomorphic VT. We analyzed laboratory and instrumental parameters at baseline and 12 months after intervention.

RESULTS

Efficiency of RFA was 85% throughout a year. Clinical improvement was associated with CHF class decrease and 6MWT distance increment from 320 [200; 340] to 411.5 [340; 461.3] m (p<0.001). NT-proBNP significantly decreased from 756 [493.5; 1106] to 269.5 [122.8; 769.8] pg/mL after RFA (p=0.006). LVEF increased from 40 [34; 46] to 47% [35; 52] (p<0.001).

CONCLUSION

RFA is effective for ventricular rhythm disturbances. This procedure improves functional status of patients with ischemic CHF and contributes to reverse LV remodeling.
