Aim. The objective of the present study was to evaluate the changes of the clinical signs and symptoms and the sonographic features in conjunction with monitoring characteristics of a coagulogram during the initial conservative treatment of deep venous thrombosis (DVT). Material and methods. The results of clinical-ultrasound and laboratory monitoring (characteristics of a coagulogram) are presented as obtained at the onset of conservative therapy of DVT of the lower extremities in 20 patients (21 cases). The treatment protocol: enoxaparin sodium 1.5 mg/kg body weight daily from day 1 to 10 followed by warfarin from day 2. The starting dose of warfarin used in 19 (95%) patients was 5 mg and in the restudying 1 (5%) patient 2.5 mg. Neither dose was changed throughout the study period. Round the clock elastic compression was applied to the open ward patients. Results and discussion. The study has demonstrated that clinical symptoms of DVT regressed within 11 days after the initiation of therapy, with mild pain syndrome persisting in 33.3% of the cases. Cyanoses disappeared in 95.2% of the patients, thigh and lower leg swelling decreased to the difference of 1 cm between the perimeters of the normal and affected extremities (p<0.0001)/ Recanalization of thrombotic masses bean on day 6 and occurred in 84.6% of the patients by day 11. Hypocoagulation and reduced blood coagulation activity without activation of fibrinolysis were observed in the end of the starting treatment. The mean values of international normalized ratio (INR) significantly increased but restudyed within sub-therapeutic limits, coagulogram characteristics were not significantly different depending on the degree of venous bed recanalization.
