Open and laparoscopic resections of the pancreas in congenital hyperinsulinism

Congenital hyperinsulinism (CHI) is characterized by recurrent episodes of hyperisulinemic hypoglycemias because of various genetic disorders. Diffuse drug-unresponsive and focal forms of CHI can be surgically treated. Nowadays there are only a few articles about maloinvasive selective resection of pancreas or pancreatectomy in children.
52 children were operated on for congenital hyperinsulinism in the department of pediatric surgery in the Almazov Medical Research Center from 2017 till April 2022, three of them were treated by laparoscopic approach.
According to results of PET/CT with 18F-DOPA in 52 children, diagnosed 12 (23.0%) diffuse forms, 33 (63.5%) focal and 7 (13.5%) atypical form. In this article authors describe 3 clinical cases of maloinvasive surgical treatment of focal CHI forms.
Our initial experience of using laparoscopic approach showed the possibility of its effective use in CHI. The results were similar with open pancreatic surgery both in achieving relief of hyperinsulinism in focal forms and in a differentiated approach to the volume of removal of the gland (enucleation or distal resection).
