Objective. The purpose of this study is to determine the safety of laparoscopic gastrectomy in patients with locally advanced forms of gastric cancer. Material and methods. Study design: a prospective, multicenter, non-randomized (streaming randomization) study. Study period: June 2016 — May 2018 in 10 oncological centers in Russian Federation and Ukraine. Inclusion criteria: patients with locally advanced gastric cancer cT2-T4a, N0-3, M0, who are indicated for gastrectomy with D2 lymphadenectomy; age from 20 to 82 years; without serious concomitant diseases (ECOG 0—1). Primary study point: the number of «large» (Clavien III—V) complications in the early postoperative period. Results. 181 patients were included in the study: 74 in the laparoscopic group and 68 in the open surgery group. There were no differences in age, gender, BMI, or preoperative laboratory parameters in patients. Conversions were performed in 6.7% of patients. Laparoscopic interventions had a longer duration of 312 min vs 233 min (CI 95% –101; –56; p<0.0001), but less blood loss of 99 ml vs 181 ml (CI 95% 44; 119; p<0.0001). The total number of complications did not differ in both groups: 21.6% vs 22%. The number of «large» complications (Clavien III—V) in the laparocsopic group was higher, but not statistically significant: 17% vs 10% (CI 95% –4.6; 18 p=0.23). There was a lesser pain severity on the 3rd day of the postoperative period (CI 95% 0.25; 1.01; p=0.0014), and a shorter bed-day duration (p<0.05) in the laparoscopic group. Conclusion. Laparoscopic gastrectomy should be performed only in specialized high-flow centers by a trained team of specialists. Further randomized trials are needed to evaluate their safety and oncological validity.
