Simultaneously with the successes and accumulation of practical experience in laparoscopic surgery, questions related to the technical features of the intervention have increasingly appeared. First of all, it concerns changes in the peritoneum. There is a close relationship between the two most important processes occurring at the site of damage or aggression of mesotheliocytes — dysfunction of the endothelium and dysfunction of the mesothelium. This relationship is most fully represented in the modern theory of epithelial-endothelial dysfunction, which integrates both of the above processes in the epithelium and endothelium of any organ. What happens in this complex system with laparoscopy? Are these changes reversible? Is it possible to prevent or, at least, reduce technogenic laparoscopic peritoneal remodeling? Experimental studies have shown that the degree of peritoneal dysfunction depends on the value of the gas pressure and the duration of the intervention (ie, the «intensity» and duration of the pneumoperitoneum). A separate consideration requires the question of the effect of laparoscopy on the peritoneum with peritonitis, because more and more laparoscopic interventions are increasingly performed in the treatment of various etiologies peritonitis. In this regard, laparoscopic interventions should be considered not only as highly effective surgical technologies, but also as an object for further fundamental scientific experimental and clinical studies.
