Clinical and economic features of surgeries for acute appendicitis are reviewed in the article. Majority of surgeries were performed for destructive forms of acute appendicitis. Among the least acute phlegmonous appendicitis were prevalent — in 184 (88%) of 210 patients per 1 year. Due to this these values were taken for comparative economic analysis. In 9% of patients appendectomy was performed through McBurney incision, in 91% of patients laparoscopic appendectomy was accomplished with ligation of appendiceal stump. Economic efficiency of endoscopic and conventional appendectomies was assessed with the use of cost/efficiency ratio. The minimum cost/efficiency ratio was taken as the most preferable as it discovered treatment modality with the least costs per efficiency unit. Laparoscopic appendectomy showed to be advantageous over conventional appendectomy because from clinical point of view it reduces length of hospital stay, number of postoperative complications and need for analgesics. From economic point of view total costs in laparoscopic appendectomy lower by 38% than in conventional one. Results presented speak for clinical and economic rationale of laparoscopic appendectomy implementation.
