OBJECTIVE

To assess clinical and economic effectiveness of screening for chronic kidney disease (CKD) using test to detect creatinine level in the blood in patients without risk factors of CKD and with type 2 diabetes mellitus (DM).

MATERIAL AND METHODS

The Markov model predict the effectiveness (quality adjusted life years) and direct medical costs for two scenarios of screening for CKD by determining creatinine level in the blood: 1) screening of patients without risk factors of CKD development; 2) screening of patients with type 2 DM. Two cohorts have been traced over a period of 10 years in each scenario: patients who have or have not been screened for CKD and incremental «cost—effectiveness» ratio (ICER) have been calculated.

RESULTS

Screening in both scenarios results in less number of fatal outcomes and more saved life years. ICER for screening of patients without risk factors of CKD development was equal 344 627.63 rub./quality adjusted life year, for screening patients with type 2 DM — 119 269.03 rub./quality adjusted life year, that is significantly less than for some used treatment methods of type 2 diabetes mellitus and cardiovascular diseases in practice.

CONCLUSION

Screening for chronic kidney disease with the determination of creatinine level in the blood is clinically and economically rational when examining patient both without risk factors and with type 2 diabetes mellitus.
