Abstract:

One of the causes of mechanical jaundice is the blockade of the bile ducts by a tumour. The most efficient method for the drainage of the obstructed biliary tract is transcapillary stenting of the extrahepatic bile ducts that is currently considered to be the «golden standard» in the treatment of this pathology. Whenever the retrograde access is impracticable, the alternative methods can be used including either percutaneous drainage under ultrasound or X-ray control or the surgical intervention. All these approaches are associated with the serious risk of hemorrhage. For example, percutaneous transhepatic drainage is known to cause bleeding in 2—3% of the cases while retrograde interventions result in hemorrhage in 1.2—1.5% of the clinical situations. The present article reports the experience with the combined minimally invasive treatment of hemobilia known to develop after percutaneous transcutaneous drainage of the biliary tract as exemplified by the management of a patient presenting with mechanical jaundice of blastomatous genesis.
