Laser eye surgery usually implies superficial corneal anesthesia. Eye surgery causes higher anxiety compared to dental or other outpatient procedures, since eyes are essential for perception of the world. Emotional stress can interfere with surgery. The patient is not able to lie quietly on the operating table and follow the recommendations. According to our data, premedication an hour before the intervention and terminal anesthesia of the eye surface in the operating theatre are sufficient for successful laser refractive surgery in 95% of cases. Patients with baseline high personal and situational anxiety comprise other 5%. Xenon inhalation in sub-narcotic dosages has certain advantages due to analgesic, neuroprotective and anti-stress effects. Xenon has no smell. This gas is easily and quickly excreted from the body, not metabolized, does not cause allergic reactions and cardiac depressive effect. We report 2 patients with baseline high anxiety undergoing excimer laser refractive surgery. Preoperative management implied inhalation of sub-narcotic doses of xenon. To analyze psychoemotional status of patients, we measured serum glucose and cortisol, as well as autonomic regulation (Varicard) and myocardial state (Cardiovisor system).
