The number of elderly patients undergoing anesthesia and surgery is being increased along with aging of population and development of medical technologies. Perioperative cognitive impairment is a common complication, especially in elderly patients. Postoperative delirium is the most severe and acute event predisposing to long-term cognitive impairment. Experimental and clinical studies in the last decade demonstrate that maintaining the integrity of blood-brain barrier is essential for homeostasis of central nervous system. Specific laboratory markers, imaging techniques and omics technologies provide us with evidence that surgery and anesthesia induce neuroinflammation associated with blood-brain barrier dysfunction, microglial activation and neuronal damage.
