Risk factors of chronic postoperative pain in cardiac surgery: mental characteristics and central sensitization

To evaluate the role of central sensitization and psychological factors in chronic postoperative pain following cardiac surgery, to evaluate the correlation between central sensitization and psychological parameters of patients.
The study included 100 patients who underwent cardiac and/or aortic surgery. All patients were preoperatively surveyed according to the Hospital Anxiety and Depression Scale, the Pain Catastrophizing Scale, the Chronic Pain Coping Inventory and the Central Sensitization Inventory. After 3—6 postoperative months, all patients were interviewed regarding chronic postoperative pain. Finally, all patients were divided into 2 groups: 30 patients with chronic postoperative pain and 70 patients without chronic postoperative pain. Statistical analysis included the Student’s t test, nonparametric Mann—Whitney U-test, Pearson’s chi-square test and Spearman’s rank correlation coefficient.
Chronic postoperative pain occurred in 30% of patients. We found significantly higher levels of pain catastrophizing (p=0.025) and anxiety (p=0.004) in the main group. There was a correlation between central sensitization and chronic postoperative pain (p=0.002). Central sensitization positively correlated with maladaptive pain coping strategies (rs=0.35, p=0.0003), depression (rs=0.5, p<0.0001), anxiety (rs=0.71, p<0.0001) and pain catastrophizing (rs=0.59, p<0.0001).
Understanding the risk factors of chronic postoperative pain will contribute to development of individual preventive programs.
