The purpose of the study was to create an algorithm for the prevention of bleeding and optimization of surgical intervention when performing laparoscopic myomectomy. Materials and methods. Examined 82 patients of reproductive age with uterine myoma. Patients of the 1st group (32 patients) underwent laparoscopic myomectomy with intraoperative administration of the synthetic analog of vasopressin — terlipressin (remestype) into the cervix, whose metabolites give pronounced vasoconstrictor and hemostatic effects. Patients of the 2nd group (50 patients) underwent laparoscopic myomectomy using an ultrasonic scalpel at the main stages of the operation. Results. In the 1st group, 108 myomatous nodes were removed, in the 2nd group — 97 myomatous nodes. The average duration of surgery in the 1st group was 137.2±49.84 min, in the 2nd group — 146.2±50.2 min (p=0.43). The volume of intraoperative blood loss in the 1st group (172.4±118 ml) was significantly lower than in the 2nd group (205±123 ml), p=0.02. Conclusion. The use of terlipressin (remestype) for laparoscopic myomectomy made it possible to remove a larger number of nodes with a larger diameter with a significant decrease in the volume of intraoperative blood loss. The use of the drug in patients with cardiovascular diseases requires a differentiated approach. The technique is simple and executable, it is indicated for any location and size of the node, does not require additional time, eliminates the need for additional hemostasis.
