BACKGROUND
Uterine leiomyoma is a benign monoclonal neoplasm of smooth muscle cells. Epithelioid uterine leiomyoma is a rare histological variant of uterine fibroids that is difficult to diagnose and treat in a timely manner, the frequency of occurrence is about 1% of all cases.
OBJECTIVE
Demonstration of clinical case of successful organ-preserving surgical treatment of large uterine epithelioid fibroids in a patient of reproductive age with further pregnancy planning.
MATERIAL AND METHODS
In this clinical case, a 41-year-old patient with reproductive plans, a history of unsuccessful laparoscopic myomectomy, due to pulling pains in the lower abdomen, an increase in size of the uterus to 16—17 weeks of pregnancy in 2024 underwent laparotomy, excision of the myomatous node, myometrectomy of recurrent epithelioid uterine fibroids large sizes.
RESULTS
During the surgical intervention, the myomatous node was peeled out, myometrectomy was performed with installation of active drainage for the outflow of serous hemorrhagic discharge, which was removed on the 3rd day after surgery. Histological examination results: morphological picture is most consistent with epithelioid leiomyoma with focal dystrophic changes. On the 5th day the patient was discharged in satisfactory condition under supervision of gynecologist.
CONCLUSION
Epithelioid uterine leiomyoma is a rare histological form of aggressive uterine leiomyoma, accompanied by a high frequency of recurrence and impaired reproductive function in women. Timely diagnosis and choice in favor of organ-preserving surgical treatment prevent the progression of this formation, having a beneficial effect on quality of life and the realization of reproductive function.
