Along with the high incidence of obstetric and perinatal complications of delivery, the use of an anesthetic technique and an early postpartum period in patients with brain neoplasms are associated with progressive neurologic symptoms, intracranial pressure changes, and a high risk for dislocation. However, delivery and the choice of anesthetic tactics in relation to the site of a tumor, its histotype and clinical course remain debatable so far. Objective — to analyze anesthetic tactics for labor and delivery in pregnant women with different types of brain neoplasms. Material and methods. A total of 103 birth records were retrospectively analyzed in patients with brain tumors, who had been followed up at the Moscow Regional Research Institute of Obstetrics and Gynecology in 2014—2017. Results. All the 103 patients successfully delivered babies. The decision was made individually about the anesthetic technique: general balanced anesthesia with mechanical ventilation or central segmental blockade with preserved spontaneous breathing. At the same time, there were a small percentage of anesthetic complications, not exceeding the statistical data on those in healthy parturient women. No maternal and perinatal deaths were recorded among the patients delivered. Conclusion. Delivery and anesthetic techniques in patients with brain neoplasms require specific knowledge, particular experience, and an individual multidisciplinary approach. The pharmacokinetic and pharmacodynamic profiles of current short-acting intravenous anesthetics, analgesics and relaxants, if properly used, provide safe anesthesia for both the mother and the fetus.
