The objective of the present study was to evaluate the cellular and humoral immune status and the cytokine profile of the blood and oral fluid in the pregnant women presenting with inflammatory diseases of the periodontium differing in the degree of severity. Material and methods. The study included 115 pregnant women of whom 30 were healthy (had no pathological changes in the periodontium), 38 with physiological pregnancy suffered chronic generalized catarrhal gingivitis, and 47 with physiological pregnancy presented with chronic generalized periodontitis. Results. The pregnant women with chronic generalized catarrhal gingivitis were shown to have the elevated levels of pro- and anti-inflammatory cytokines in the oral fluid in the absence of alteration of the blood cytokine status. In the women with physiological pregnancy and chronic generalized periodontitis the levels of cytokines were elevated both in the blood and in the oral fluid. These two clinical conditions were associated with unidirectional changes of cellular immune reactions including the general reduction of the lymphocyte population, the decrease of the relative amount of mature T-lymphocytes and T-helpers in conjunction with the increase in the relative number of natural killer cells. Conclusion. Changes in the levels of cytokine mediators of inflammation in the pregnant women presenting with chronic generalized catarrhal gingivitis are confined to the oral fluid whereas women with chronic generalized periodontitis exhibit the systemic inflammatory response characterized by the overall alteration of the blood cytokine profile.
