Endometriosis is a gynecological disease characterized by the presence of tissue with morphological and functional properties similar to the endometrium outside the uterine cavity. This pathology affects about 10% of women of reproductive age worldwide and currently has no established etiology. However, the hypothesis of immune dysfunction as the main cause of endometriosis is quite controversial among researchers around the world. It has now been established that patients with endometriosis experience dysfunction of the cellular and humoral immune systems, consisting of improper functioning of signaling pathways, an imbalance of pro-inflammatory and anti-inflammatory cytokines, impaired reception and intercellular interaction, activation of an autoimmune inflammatory response and tissue damage. In addition, it is known that these patients have polymorphism of genes responsible for the development of a number of autoimmune conditions, which, together with the fact of production of autoantibodies, indicates a possible connection between endometriosis and autoimmune pathologies. The purpose of the review is to summarize modern scientific data on the problem of immune dysregulation in the etiology of endometriosis and describe the abnormal immunological mechanisms involved in its pathogenesis, as well as to identify autoimmune conditions comorbid with endometriosis, to study the possibilities of using immunological and immunogenetic biomarkers for early non-invasive diagnosis of endometriosis and the possibility of targeted immunotherapy, immunomodulation for the purpose of further personalized approach to patient data management
