The aim of investigation was a systemic analysis of the contemporary literature data to assess the efficiency of new approaches to diagnosis and treatment of endometrioma. The review presents the data of home and foreign articles included into PABMED and RINTS bases and published within the period of 2017—2019 years. The up-to-date principles of the therapeutic strategy were summarized as realizing the transition from histological to clinical diagnostics, rationality and personification of surgical and medicamental treatment which must be chosen according to the female-patient age, the severity of the disease and her desire to reserve fertility as well as providing the balance between the supplementary advantages, risk of damages and the cost of medical interference. The novel paradigm of diagnosis and treatment of endometriomas is summoned to improve the quality of medical care based on principles of medical aid personification. Subsequent progress is connected with interdisciplinary medical research of molecular basis of biology and pathophysiology of endometriosis, the search for non-invasive biomarkers and their validation which will help to create new therapeutic trends and highly effective individual plans oriented on the concrete female patient.
