Objective. To improve the efficiency of chronic pain syndrome treatment in women with external genital endometriosis by the comparative analysis of the results of postoperative complex treatment with dienogest 2 mg and gonadotropin-releasing hormone agonists (aGnRH) (goserelin) 3.6 mg in combination with psychotherapeutic methods. Material and methods. Comparative analysis of the results of 131 patients treatment was performed. Group I consisted of 88 patients with external genital endometriosis of II—IV stages (AFS classification, 1986), group II consisted of 43 women without external genital endometriosis (control group). All women were surveyed for intensity of pain syndrome, level of depressive-anxiety disorders, state of health, activity, mood, and quality of life. The surgical treatment of all patients (n=131) had been performed laparoscopically. Postoperatively, women in group I (n=88) were randomly divided into two subgroups for hormone therapy: subgroup IA — 49 (55.7%) women who were prescribed a 2 mg dose of dienogest; subgroup IB — 39 (44.3%) women who were prescribed 3.6 mg dose of aGnRH (goserelin), for a period of 6 months, in conjunction with psychotherapy. Results. The complex treatment showed positive dynamics in the clinical picture and the psychological condition of women with EGE in comparison with the baseline data. Conclusions. Dienogest and aGnRH in combination with psychotherapeutic methods were equally effective in treating women with chronic pain syndrome associated with external genital endometriosis. However, the drug of choice for treatment of women with chronic pain syndrome with external genital endometriosis is 2 mg dienogest.
