OBJECTIVE

To evaluate accumulation of gentamicin and vancomycin in peripheral blood and purulent focus in patients with phlegmon of the maxillofacial area.

MATERIAL AND METHODS

The clinical study included patients with phlegmon of the maxillofacial area. These ones were divided into 2 groups by 10 people depending on antibacterial drug (gentamicin and vancomycin). We analyzed accumulation of both drugs in the area of purulent-inflammatory process considering concentration of these drugs in blood of patients with phlegmon of the maxillofacial area. We compared concentrations of gentamicin and vancomycin in peripheral blood (ulnar vein) and area of purulent inflammation (facial vein).

RESULTS

We found higher concentration of antibacterial drug in the inflammatory zone due to higher microvascular permeability and blood rheology changes (blood thickening and slowing down). Thus, the drugs infiltrate tissues through expanded endothelial fenestrae and pores.

CONCLUSION

We found higher concentration of gentamicin and vancomycin in the inflammatory zone compared to peripheral blood.
