Abstract:

Objective — to study the possibilities of treating patients with pseudomyxoma peritonei, by using cytoreductive surgery and various intraoperative antitumor procedures. Subject and methods. The paper presents the experience of combined treatment in 65 patients with pseudomyxoma peritonei of appendicular origin. Surgery was supplemented with intraoperative photodynamic therapy (PDT) in 32 (49.2%) patients; hyperthermic intraperitoneal chemotherapy (HIPC) was performed in 25 (38.4%); 3 patients received intraoperative aerosol chemotherapy and 5 had surgical treatment only. Results. The value of peritoneal carcinomatosis index, the completeness of cytoreduction, and the volume of surgery were analyzed. Postoperative complications were 16.9% with an in-hospital mortality of 4.6%; the 5-year survival rates were 86.6 and 65.2% in the HIPC and intraoperative PDT groups, respectively. Conclusion. Combined surgery is a highly effective treatment for patients with pseudomyxoma peritonei on the assumption of careful patient selection.
