Objective. Тo increase the efficiency of radiation therapy in patients with malignant tumors of the trachea and lung with endobronchial radiotherapy (EBRT). Subjects and methods. The results of radiation therapy in 101 patients receiving EBRT as both a radical or palliative treatment option to preserve the tracheal and/or bronchial lumen were presented. Primary and recurrent cancer of the trachea and that of the bronchus were treated in 50 and 51 patients, respectively. Results. In the palliative treatment group, all the patients with tumors of the trachea could preserve its lumen for adequate lung tissue ventilation. In the radical treatment group, 29 (80.6%) of 36 patients were noted to have complete tumor resorption. Three-year relapse-free survival rates were 83.3±6.2%. Five-year overall survival rates were 66.7±7.9%. In the group of radical treatment for lung cancer, 12 (56.5%) of 23 patients achieved complete tumor resorption; three-year relapse-free survival rates were 43.5±10.6%; five-year overall survival rates were 34.8±10.2%. Conclusion. Independent EBRT as a component of combined radiation or combined treatment expands indications for radical and palliative radiation treatment, including that in patients who have been considered incurable, provides a stable local control and quality of life in patients.
