The paper gives a clinical example of successful treatment in a patient with metastatic melanoma and cholangiocellular carcinoma of the liver with further melanoma progression as solitary brain metastases. Clarifying diagnosis in the patient with melanoma could reveal the second tumor — Klatskin tumor whose radical removal permitted the patient to survive 5 years without any progression. Twenty-eight months later, there was melanoma progression as solitary intracerebral foci. Control of the latter could be achieved by a comprehensive approach that involved surgical treatment with a photodynamic therapy session, radiosurgery, and temozolamide therapy. Following 20 months, control examination (brain MRI and methionine positron emission tomography) revealed no recurrences in both the area of surgery and that of brain radiation. Planning examinations have shown no extracranial metastases from liver cancer and melanoma so far. This observation suggests that it is expedient to choose aggressive treatment policy for such patients.
