Objective — to develop and improve endobronchial treatments for early central lung cancer (CLC). Subjects and methods. In 1984 to 2014, the P.A. Herzen Moscow Oncology Research Institute developed and clinically used endobronchial procedures for surgery and photodynamic therapy (PDT) in 125 patients. Early bronchial carcinoma was a single tumor in 45% of the patients; two or more foci of CLC were present in 55%. White light video endoscopy, autofluorescence, alasense-induced fluorescence (PhotoDynamic Diagnosis (PDD)), narrow-band imaging (NBI) bronchoscopy, local fluorescence spectroscopy (LFS), adaptive digital endoscopic spectroscopy estimating the index of Hemoglobin (iHb), as well as immunocyto- and histochemical ((MUC1, Tp53, VEGF) studies were employed to diagnose early CLC. Electric laser destruction, argon plasma coagulation, and PDP were used to treat patients. Results. Complete regression of early CLC could be achieved in 89% of cases and that of tumors less than 1 cm was in 100%. A recurrence was diagnosed in 17% of cases. Conclusion. The developed procedures of endobronchial surgery make it possible to perform radical treatment in patients with recurrent CLC and to increase the number of cured patients with a poor somatic health status.
