The results of pneumonectomy for locally advanced lung cancer were analyzed in 71 patients. Pneumonectomy with circular resection of the tracheal bifurcation was performed in 36 patients; standard extended pneumonectomy was carried out in 35 patients. The frequency of complications and mortality rates were 35.21 and 19.72%, respectively. Both groups showed no significant differences in the values of complications (p>0.1) and mortality (p>0.5). The median survival was 12.24 months; in Group 1, three-year survival rate was 22.8±9.9% and five-year survival rate was 7.6±9.4% (one patient survived 5 years). In Group 2, these were 26.5±8.0 and 21.4±7.9%, respectively (4 patients survived 5 years). There were no significant differences in survival rates between the groups (p=0.6227). The immediate and late results of pneumonectomy with circular resection of the tracheal bifurcation did not differ from those of extended pneumonectomy, therefore it is a quite warranted surgical procedure in the treatment of locally advanced lung cancer.
