The review of modern medical literature demonstrates the steadily increasing interest of researchers in anatomical lung segmentectomies. The main question, which has remained relevant for decades, is the oncological feasibility of anatomical segmental resection. The overwhelming number of studies indicate to the validity of this volume of surgery in radical surgical treatment of patients with stage IA1 and IA2 non-small cell lung cancer, i.e., when the tumor size is less than 2 cm without regional metastases. Nevertheless, the discussion remains open. We present a clinical case of long-term recurrence-free follow-up of a patient after anatomical resection of segments I — III of the upper lobe of the left lung for cancer.
