Abstract:

OBJECTIVE

To identify the main organizational problems in medical care for peripheral artery disease and propose possible solutions considering professional opinion of cardiovascular surgeons.

MATERIAL AND METHODS

In 2021, 105 cardiovascular surgeons in St. Petersburg hospitals were surveyed about organization and availability of medical care for patients with peripheral artery disease. Intensive and extensive indicators were calculated with assessment of differences. Correlation analysis was carried out.

RESULTS

Doctors rated organization of specialized care by 4.1 points, primary health care — by 2.5 points in a 5-point system. The main causes of unsatisfactory work of polyclinics (per 100 surveyed doctors) were understaffing of cardiovascular surgeons (70.6), low qualification of doctors (47.1), long waiting for diagnostic tests (35.3). Low patient activity was also noted (per 100 surveyed doctors): delayed help-seeking (82.4), low health literacy (41.2), non-compliance (39.2). Follow-up of patients with peripheral artery disease requires a multidisciplinary approach, but only 25.1% of patients are followed-up.

CONCLUSION

Systemic organizational problems in medical care are caused by low availability of primary health care. Certain approaches to their solution are proposed.
