Despite the new possibilities of adjuvant therapy of skin melanoma, timely diagnosis of this tumor remains a key prognostic factor determining the overall survival. The article describes a series of cases of skin melanoma smaller than 6 mm in diameter. Clinical, dermatoscopic, and histological manifestations of early melanomas differ from those characteristic of this malignant tumor. Thus, all neoplasms are bicolored rather than polychromic. The signs characteristic of melanoma, such as irregular shape and uneven coloring, occur in only 67 and 58% of cases, respectively. The mean value of the dermatoscopic algorithm for the diagnosis of melanoma ABCD was 4.2 points, 7-point Argenziano algorithm — 2.2 points. The sensitivity of these algorithms was 16 and 50%, respectively. Distinctive features of histological picture included poor plymorphism of the nuclei, lack of growth peculiarities characteristic of the most common forms of melanoma, insignificant pagetoid growth, and the absence/low mitotic index. Early diagnosis of skin cancer is currently impossible without a comprehensive evaluation of skin lesions with allowance for the clinical presentation, dermatoscopic examination data, and the results of histological examination, and therefore requires an interdisciplinary approach.
