Basal cell carcinoma is the commonest cutaneous malignant neoplasm in humans. The scientific literature describes a large number of criteria for pigmented basal cell carcinoma whereas the dermatoscopic characteristics of the non-pigmented tumour are poorly known. The objective of the present study was to elucidate the main dermatoscopic features of pigment-free basiliomas. We identified 111 basal cell carcinomas based on the results of clinical observations, dermatoscopic images, and cytological investigations including 5 (4.5%) pigmented and 106 (95.5%) non-pigmented tumours. Our findings indicate that 70 (66%) basiliomas display bright homogeneous regions of white and red colour, 66 (62%) eroded sites and ulcerations, 56 (53%) short and thin teleangiectasias, 46 (43%) dendriform vessels 33 (31%) grey-blue globules, 25 (24%) leaf-shaped regions, and 25 (24%) polymorphous vessels represented by uneven lines, points, and pins. It was shown that bright white- and red-coloured homogeneous zones as well as ulcerations are the main dermatoscopic signs of non-pigmented cutaneous basal cell carcinoma. Vascular structures of these tumours are represented largely by branching capillaries (short and thin teleangiectasias) in the superficial form and  dendriform vessels in the nodular form.
