Objective. To analyze the early outcomes of concomitant heart valve surgery and Cox Maze IV procedure. Material and methods. Heart valve surgery with concomitant Cox Maze IV procedure was performed in 49 consecutive patients for the period from January 2016 to December 2019. There were 23 (46.9%) mean. Age of patients ranged from 16 to 74 years (mean 61.3±9.9 years). Left atrial dimension ranged from 3.8 to 8.8 cm (mean 5.7±1 cm), left atrial volume — from 80 to 820 ml (mean 215.7±128.5 ml). Most patients had mitral and tricuspid valve disease (n=23, 47%) or mitral valve disease (n=13, 26.7%). Paroxysmal atrial fibrillation (AF) was diagnosed in 8 (16.3%) cases, persistent AF — 19 (38.8%) patients, longstanding persistent AF — 22 (44.9%) cases. Results. One (2%) patient died from the causes not related to Cox Maze IV procedure. Temporary cardiac pacing in early postoperative period was required in 27 (57.4%) patients. There were no cases of permanent pacemaker implantation. At discharge, 37 patients (77.1%) maintained sinus rhythm, other 11 patient had AF. Mean length of hospital-stay was 10.2±3.6 days (range 7—27 days). At discharge, sinus rhythm was restored in 7 (87.5%) patients among those with paroxysmal AF, in 16 (84.2%) patients among those with persistent AF and in 14 (66.7%) cases among patients with longstanding persistent AF. Conclusion. Cox Maze IV procedure in patients with valvular heart disease is an effective treatment of AF with low postoperative morbidity and mortality.
