Objective. Retrospective analysis of conventional biatrial maze (cut-and-sew) procedure and its left atrial variant in the treatment of atrial fibrillation in patients undergoing concomitant cardiac surgery. Material and methods. Biatrial and left atrial maze procedures were made in 217 (group 1) and 176 (group 2) patients for the period from January 2013 to December 2018. Concomitant procedures included coronary artery bypass grafting and/or heart valve surgery. The results were compared at discharge and within 12 months after surgery. Results. In-hospital mortality was 3,7% (n=8) and 2,8% (n=5) in groups 1 and 2, respectively (p=0.642). The main problem of early postoperative period was sinus node dysfunction. This event was diagnosed in 161 (74.2%) and 96 (54.5%) patients of the groups 1 and 2, respectively (p=0.001). In-hospital implantation of permanent pacemaker was performed in 20 (9.6%) and 13 (7.6%) patients, respectively (p=0.499), within 12 months after surgery — in 13 (6.7%) and 4 (2.3%) patients (p=0.075), respectively. Sinus rhythm at discharge was in 151 (72.2%) and 135 (78.9%) patients, respectively (p=0.133). Sinus rhythm after 12 months was observed in 66.4% and 75.2% of patients, respectively (p=0.172). Rhythm disturbances including recurrent atrial fibrillation after 12 months were diagnosed in 38 (18.2%) and 23 (13.4%) patients, respectively (p=0.353). Conclusion. Both groups showed comparable results regarding postoperative restoration and control of sinus rhythm. Predictors of sinus node dysfunction were biatrial technique and duration of atrial fibrillation.
