Coronary artery bypass grafting (CABG) under cardiopulmonary bypass (CPB) and cardioplegia remains the gold standard of surgical coronary revascularization in patients with multiple-vessel coronary artery disease. Off-pump CABG (OPCAB) was introduced in the early 1990s. This technique was very attractive over the next decade due to potential avoiding some complications of CPB including systemic inflammatory response and immunosuppression. No need for aortic cannulation is expedient to decrease incidence of stroke and other embolic events. However, indications for OPCAB are still unclear despite long history of this approach and numerous available trials comparing conventional CABG and off-pump technique.
