Abstract:

Aim. To evaluate the effectiveness of complete course of training program for patients with artificial heart valves to increase adherence to treatment and quality of life in 5 years after cardiac surgery. Material and methods. Two groups of patients were designed. There was a complete course of training program including inpatient and outpatient stages in the main group (n=126). The control group (n=58) consisted of patients who successfully underwent only inpatient training stage. Adherence to treatment and quality of life were analyzed before training, at the inpatient and outpatient stages and in 5 years after surgery. We assessed integral indicator of adherence to treatment by S.V. Davydov questionnaire, quality of life was estimated by SF-36 questionnaire. Results. There was 1.4-fold increase of adherence to treatment after complete course of training in the main group while it was the same in case of inpatient program alone. In 5 years after cardiac surgery similar adherence to treatment was observed in the main group while in the control group adgerence was decreased by 1.5 times. During entire follow-up quality of life was predominantly improved due to psychological health component. There was a correlation of physical and psychological health components with integral value of adherence to treatment throughout follow-up. Conclusion. The application of training program including inpatient and outpatient stages increased adherence to treatment and improved quality of life with 5-year follow-up.
