Aim. To evaluate an efficacy of monotherapy with antiarrhythmic drugs including propafenone (propanorm, PRO MED, Prague), sotalol and verapamil in comparison with control group without medicines to prevent recurrent AF and/or new atrial tachyarrhythmia in early postoperative period after catheter isolation of pulmonary veins. Material and methods. Patient’s diary, ECG, Holter-ECG and subcutaneous cardiomonitor were used to detect asymptomatic arrhythmias. 243 patients were randomized into 4 groups: group I - verapamil retard 240 mg/day (n = 61), group II — propafenone 450 mg/day (n = 62), group III — sotalol 160 mg/day (n = 60) and group IV (n = 60) without medicines. In group I mean number of electric cardioversion in 3 months after surgery was 1.016 ± 1.74 (p = 0.607), in group II — 0.40 ± 1.03 (p = 0.0096), in group III — 1,033 ± 1,52 (р = 0,152) compared with control group. Mean number of hospitalizations within 3 months was 0,654 ± 0,74; in group I - 0,684 ± 0,73 (p = 0,178), in group II (propafenone) — 0.447 ± 0.57 vs. 0.894 ± 0.88 (p = 0.0016) in the control group and in group III (sotalol) - 0.592 ± 0.67 (p = 0.044). Conclusion. Significant decrease of AF paroxysms (p = 0.032), need for cardioversion (p = 0.0096) and therefore cardiac rhythm disturbances-related hospitalizations (p = 0.0016) was observed only for propafenone in order to prevent recurrent AF in early postoperative period. None of medicines has confirmed an efficacy to prevent recurrent atrial tachyarrhythmias with regular atrial cycle in early postoperative period after pulmonary veins isolation.
