Aim - to summarize an experience of surgical treatment of aldosteroma. Material and methods. Comprehensive diagnosis and surgical treatment of 18 patients (13 (72.2%) women and 5 (27.8%) men) with aldosteroma were analyzed. All patients underwent laboratory and instrumental examination (blood K+ and Na+, hormones, sonography and CT) to confirm the diagnosis. Results. The classic symptom was hypertension of various severity in all patients. The sensitivity and specificity of sonography were 92 and 86% respectively. Those values for CT were 100 and 99.7% respectively. It was found that long-term preoperative management with spironolactone, potassium and antihypertensive drugs is advisable only in patients with hypertension type III and severe changes in target organs (heart, kidney, brain). Adrenalectomy via thoracophrenolumbotomy and mini-lumbotomy was performed in 6 and 12 patients respectively. Minimally invasive approach significantly reduced the incidence of postoperative wound complications and hospital-stay. In long-term period after adrenalectomy stable hypotensive effect was achieved in 15 (83.3%) patients. Conclusion. Surgical treatment of aldosteroma should be carried out in specialized clinics with sufficient experience in this surgery. Minimally invasive approach significantly reduced the incidence of postoperative wound complications and hospital-stay.
