Introduction. Currently, carotid endarterectomy (CEA) and carotid stenting (CAS) demonstrate the similar results in cerebral revascularization. Minor complications of surgery increase labor costs of medical personnel in the postoperative period and should also be taken into account in interventions planning. Material and methods. The study included 1826 patients who underwent surgery for internal carotid artery stenosis (ICA). Group 1 consisted of 1018 patients who underwent CEA, group 2 enrolled 808 patients after CAS. Patients in group 2 were older, more likely to suffer from coronary artery disease (CAD), diabetes, chronic obstructive pulmonary disease (COPD) and stroke history was revealed more frequently (p<0.05). Results. There were similar mortality rate, incidence of uncontrolled hypertension and transient ischemic attacks in early postoperative period. Stroke incidence was higher in patients after CAS 35 (4.3%) versus 27 (2.6%) patients in CEA (p=0.02), frequency of myocardial infarction — after CEA 38 (3.7%) versus 13 (1.6%) patients in CAS (p=0.008). Incidence of uncontrolled postoperative hypertension was similar. Conclusions. Both methods have similar safety but require an individual application for each patient.
