Abstract:

To evaluate the immediate results of mitral valve replacement in patients older than 65 years. From 2000 to 2013 mitral valve replacement (MVR) was performed in 107 elderly patients (mean age 68.0±2.8 years), including 28 of them (26.2%) - over 70 years. In the etiology of defects degenerative lesions dominated - 65 (60.7%) patients, rheumatic etiology was observed in 30 (28.1%) patients, ischemic mitral insufficiency - in 12 (11.2%) cases. Isolated mitral valve replacement was performed in 57 (53.3%) cases, in combination with tricuspid valve plasty by De Vega - in 38 (35.5%) patients. Coronary artery bypass grafting was further arranged in 12 (11.2%) patients, thrombectomy of the left atrium - 18 (16.8%) cases. Biological prostheses have been used in 28 (26.2%) cases, mechanical - in 79 (73.8%). The mortality was 3 (2.8%) patients. Non-lethal complications were noted in most patients. Satisfactory results can be achieved at the present stage in surgical treatment of mitral valve defects in patients older than 65 years. These results are comparable with the results of surgical treatment of younger patients.
