Impact of visceral fat hormones (adiponectin, leptin, resistin) and atherogenic dyslididemia on risk of stenotic complications after coronary artery bypass grafting (CABG) was studied in patients with metabolic syndrome (MS). 102 patients with III-IV functional class angina associated with MS were included in the study. Patients were divided into 2 groups depending on presence or absence of MS: group 1 (n=59) with MS, group 2 (n=43) - without MS. All patients underwent planned CABG. Lipid profile and levels of visceral fat hormones were evaluated before operation. After one year of observation stenotic complications were revealed in 28.8% of patients with MS according to data of multispiral computer tomography. According to discriminant analysis total cholesterol, low density lipoproteins (LDL), apo-lipoprotein A1, leptin, adiponectin and total cholesterol/high density lipoprotein (HDL) and LDL/HDL indices were prognostic factors for stenotic complications in patients with MS. ROC-analysis showed that levels of leptin higher >11.9 ng/ml, Apo AI < 1.2 g/l, adiponectin <14.1 mkm/l and total cholesterol/HDL index>4.8 are the most significant risk factors for stenotic complications after CABG. It is drawn a conclusion that patients with coronary heart disease and MS present with higher levels of atherogenic lipids. Significant relationship between levels of visceral fat hormones and atherogenic dyslipidemia was estimated. It is shown that certain indices of lipid profile and visceral fat hormones are prognostic factors for stenotic complications after CABG.
