Primary cancer of the trachea and bronchi is an extremely rare pathology that creates great difficulties for treatment. The quality of life during the treatment period suffers sharply due to severe respiratory failure. Stenting of the tracheobronchial tree using rigid bronchoscopy is an advanced and highly effective method. In the area of the bifurcation of the trachea, it is customary to use silicone tracheobronchial endoprostheses, which give good results in cicatricial stenosis. However, the localization of the tumor process at the level of the bifurcation with the involvement of both the trachea and the main bronchi creates difficulties for recanalization of their lumen. The convenient design and ease of use of the metal self-expanding Y-shaped stent allows it to be tightly fixed in the lumen of the tracheobronchial bifurcation, which was extremely important and necessary in this clinical case. We present the first experience of using a self-expanding metal Y-stent in the Sklifosovsky Research Institute for Emergency Medicine in a patient with a neoplasm of the tracheal bifurcation.
