Abstract:

The purpose. Evaluation of the impact of various types of approach for laparoscopic appendectomy for diffuse appendicular peritonitis on treatment outcomes. Material and methods. The study included the results of treatment of 231 patients with acute appendicitis complicated by diffuse appendicular peritonitis. The main group consisted of 152 patients who underwent laparoscopic appendectomy from the «triangulation» approach, the control group — 79 patients whose laparoscopic appendectomy was performed from the «sectorization» approach. Results. Laparoscopic appendectomy was performed in 231 patients with diffuse appendicular peritonitis. Approach «triangulation» was used in 65.8% of patients, «sectorization» in 34.2%. The average age of the patients was 43.4±18.4 years, the duration from the onset of the disease was 36.30±20.88 hours. In 200 (86.6%) patients diffuse peritonitis was revealed, in 31 (13.4%) — general peritonitis. Among patients of both groups, no fatal outcomes were noted. With a comparable severity of peritonitis, there was a significant predominance of intra-abdominal postoperative abscesses in the group of patients operated by «sectorization» approach (2.6% versus 13.9%; p=0.0017). Conclusion. Laparoscopic interventions for widespread appendicular peritonitis using the «triangulation» approach lead to a decrease in the incidence of postoperative intra-abdominal abscesses.
