Abstract:

Significance of acute appendicitis is owed by high morbidity as well as diagnostic pitfalls at all stages comprising 12-35% of cases. Diagnostic mistakes account for removal of microscopically normal appendices in 20-45% of cases from one side and delay in surgical treatment and development of complicated acute appendicitis markedly affecting on patients quality of life. Study was aimed to improve diagnosis and treatment of acute appendicitis by means of learning of its clinical implications and diagnostic features reliance on appendiceal morphological changes. 1729 patients with acute appendicitis were under supervision. Upon that destructive structural changes of the appendix were revealed in 41.4% patients. Laparoscopy was performed in 142 patients. Among them in 21 (14.7%) patients under local anesthesia and in 121 (85.3%) patients under general anesthesia. Laparoscopy was finished at diagnostic stage in 38 (26.7%) patients, destructive appendicitis was diagnosed in 96 (67,6%) patients. Other surgical diseases of abdominal cavity organs were discovered in 8 (5.6%) patients. Thus 67 (47.1%) patients managed to avoid more traumatic surgery. In all cases diagnostic laparoscopy made things clear, there were no divergences in clinical and morphological diagnosis. Whereas refusal of diagnostic laparoscopy showed overdiagnosis of destructive appendicitis in 18% of cases, underdiagnosis - in 6.9%.
