Abstract:

Surgical treatment of perforated gastroduodenal ulcers up until now remains one of the most frequent urgent surgeries on stomach and duodenum associated with significant postoperative complications and high mortality despite achievements in antiulcer and antihelicobacter therapy. From June 2009 to December 2012 42 surgeries for the perforated gastroduodenal ulcers were performed in a surgical clinic of the Reutov central city clinical hospital. Among them in 27 (64.2%) cases laparoscopic suture of the perforation was accomplished. Average duration of the conventional surgery was 91±37 min, laparoscopic surgery - 111.8±39.4 min. Rate of suture insufficiencies was higher in group of patients operated laparoscopically, with overall complications rate being higher after conventional surgeries (р<0,05). The cost of laparoscopic procedure for the perforated gastroduodenal ulcer exceeded that of conventional surgery by 8%. Initial statistic results obtained proved feasibility of laparoscopic perforation suture with satisfactory results compared with the conventional surgeries in terms of municipal health-care institution. Economically, the cost of laparoscopic surgery is higher than that of open one. However, reduction of hospital stay and cost cutting in treatment of postlaparoscopic complications allow laparoscopic management of the perforated gastroduodenal ulcers to have hopeful prospects.
