Laparoscopic surgeries traditionally considered as less traumatic are followed by moderate-to-severe postoperative pain syndrome. Its intensity on the first postoperative day may exceed that after open surgery. Severe pain syndrome per se is a serious obstacle for early postoperative rehabilitation within the concept of fast-track surgery or ERAS (Enhanced Recovery After Surgery). Modern foreign guidelines for anesthesia during laparoscopic surgery are not perfect and not always applicable in national practice. We have developed our own anesthesia and analgesia schemes for some types of laparoscopic procedures after comprehensive analysis of modern foreign recommendations and evidence-based medicine data. The schemes include such drugs as dexmedetomidine, subanesthetic doses of ketamine, lidocaine (I/V infusion), gabapentin, as well as skin infiltration with long-acting local anesthetics prior to incision.
