It is well known that compared to young patients, people over 60 years have a higher risk of developing adverse postoperative outcomes, due to age-related changes in physiological functions, the presence of several concomitant diseases, polypharmacy, cognitive dysfunction and specific geriatric syndromes such as frailty. More than a quarter of this category of people have several chronic diseases of the cardiovascular, nervous, endocrine, hematologic, musculoskeletal, respiratory, digestive systems. In the process of aging, there is also a decrease and some distortion of metabolic processes, a decrease in the functional reserves. All of the above, along with the amount of surgical intervention increases the risk of postoperative complications and perioperative mortality. The purpose of developing these recommendations is to summarize the current data on perioperative management of elderly patients, they include provisions for preoperative evaluation, including neuropsychiatric assessment, necessary diagnostic measures, the choice of the method of anesthesia, the specifics of its conduct and measures for prevention of perioperative complications.
