In a series of 128 consecutive patients with nasal septum perforation surgery plastic repair of the perforation was attempted in 91 cases. Among them, endonasal approach and local flaps were used in 75 cases, additional buccogingival flap - in another 4, open rhinoplasty approach was used in the rest 9 cases. Complete closure of the perforation was achieved in 66 (72.5%) patients, 24 (26.4%) of them reported reduction of the size of perforation, in one case (1.1%) only size of perforation slightly increased. Subjective and objective results of the surgery in this series did not depend on traumatic or non-traumatic etiology of perforation. Details of surgical technique and postoperative management of post-traumatic and so-called spontaneous perforations are reported, and possible causes for the perforation recurrence, particularly contamination of the nasal cavity with S. aureus are discussed.
